The piece was standing on its end, the domi was sitting flat and plunging into the vertical workpiece.
Should have plunged only half way through but forgot to reset depth. My hand was holding the vertical workpiece to the domi. I plunged clean through. All this with pics was in the previous thread. Dumb thing to do, totally.

I don't understand. As I said, there was wood behind the triangles The reference surface was the bench, not the fence.The fence was in the up position.

Having put my embarrassment aside, here are some images of the damaged screw and cutter.
Yep Waldo. One (actually two) stuffed cutters. One was recovered by diamond filing the end cutting edges, the other could not be recovered.
For those who don't already know, domino cutters can be resharpened (end cutters only) a little because the domino machine places a slightly longer cut into the workpiece. So taking up to a mm off the end of the cutter should still give you deep enough holes - just.
