I wonder how many of these guys were labelled idiots back in the day?
Yes, many died and thats how stuff got done back then... Rhetorical questions but, did they have a WHS rep? Did they have a choice? Did they have to work so their families could eat? Did the boss say if you dont want to do it I have 20 other men in line to take your job...
Context is important, as is perspective.

    Fallen Woodwork

I have always been staggered by that pic, but I read recently that it was not the norm, at least for smoko and lunch breaks, but was staged for the photo op. If that was me, the food would have gone straight through! However, their general work practices would have not been dissimilar and your rhetorical questions are probably all on the money (unintentional pun) as the depression was looming in the early thirties.

    It was a different time.

Little doubt it was staged but that image is what I was reminded of after reading Mobyturns points and illustrates a few others.
I wonder if any of them did wear a safety line during normal work. Yep , lunch would not be going the right direction if I were on that girder.
I'm vertical rescue qualified, we are always attached to two independent anchor points if going over an edge.
Good pick up on the depression. Some people are doing it tough right now and if what I read is correct, it isnt going to get better any time soon. Might make a few more idiots out of us
