When making handles I always use wooden wedges, and I prefer to cut the slot for the wedge's closer to the edges rather than in the middle of the handle . same as through stool legs in seats or in rails. I like to cut it so the wood can flex out towards the head rather than crack or split like what you can see when its in the middle.
Ive never seen a head come loose with wooden ones , they are glued as well of course .
I have seen plenty of loose steel ones though.
