    Old school flat shank and blade screw driver rather than a round shank with a flat forged/ground on the end.
    PS also it can be a thread that gets turned akin to a tread in a car jack etc.

    I have seen the flat shank of a turnscrew gripped tightly with an adjustable wrench to give extra turning power while pushing down with the other hand on the top of the handle.
    Mighty useful in removing stubborn screws.
    Can't do this with a round shank screwdriver.

    Here's a similar archaic expression. In one of Jerome K Jerome's books (Three Men on the Bummel I think) I came across 'screw hammer'. From the context it was a spanner.
