See Attached forms you can click on them and save the form. It is a word document and you should be able to just click the boxes and fill it in.
Fill in the smaller sections first, Phone, Email, Post code as the fields may move if you enter a long name or address first.

For some reason it is having trouble the Word document.

I can't see any form that has fields to fill in, so I guess you are still having difficulty with the word attachment.
