See Attached forms you can click on them and save the form. It is a word document and you should be able to just click the boxes and fill it in.
Fill in the smaller sections first, Phone, Email, Post code as the fields may move if you enter a long name or address first.

You can email me or phone me to arrange payment, contact details at at the bottom of the form, the form can be e-mailled.

Don't forget places are limited and your place is only secure once the registration form & payment are received.

For some reason it is having trouble the Word document.
The PDF version below doesn't have the check boxes and needs to be done manually. I can email the word doc to those who want to use that version.

Hi Dale

I can't see any form that has fields to fill in, so I guess you are still having difficulty with the word attachment.

However, in the interim I will confirm my attendance, and also that of ozhunter (Adam Brown).

I have emailed the word version to you, or you can use the pdf and rescan it.

We are looking at 20 places. So far NSW is leading the charge with enrolments.

Hey Joel, after you get the email (or when Dale says he has noted your address) then edit your post to delete the emaill addy....or look forward to LOTS of spam.....

Took a lot of work to override the security but by now DSel should have the application and the payment
