Steve the holes are for some sort of spanner/wrench But you shouldnt need one, as you rotate the work piece clock wise it will tighten up. OK you do need a cutter , but Mcjings dont sell them with a cutter , but you can pick up a 60mm Forsnter bit. If you can find a imperial size I think its 2-5/16. Bruce designed this chuck for secondary schools and it was in service for a great number of years. I have one and a ripper of a chuck, wouldnt part with it , so much so I have been thinking of making a small one around 30mm and or buying another from McJings
