Just as a bit of info. It is running at about 700rpm, about 12 revs/second.

I think Pipeclay is correct here, the CCMT insert is designed with a 7deg relief angle and a positive tip. Turning it over may not be a good idea, and I don't think the fixing hole in the insert is...

The "mirror finish" tends to be on aluminium, (using WD40 as a cutting fluid), I wouldn't worry about it on steel, so long as it is reasonably flat that's all one needs.:)

My opinion is there is nothing wrong with the bearing(s)
If the bearing(s) were to make a noise like that, you would notice the play in the spindle.
